CNA on Med/Surg. She is twentythree
years old and lives in Donnellson.
Rachel has been married for three years
and has a one-year-old son. She plans
to continue her education in nursing.

FAILURE TO ENROLL BEFORE
JANUARY 15, 2011 WILL RESULT IN
CANCELLATION OF ALL
INSURANCE PLANS.

make an appointment.
If HR is not open during your
shift, leave a message with a good time
and number for HR to return the call. We
will schedule early or late appointments as
needed.
Although employees have until
January 15th to enroll, failure to enroll
before January 1st may cause:
adjustments to your paycheck to catchup
premium deductions, delay in
receiving insurance cards from all
carriers, and delay in being in the
insurance carriers system, which may
cause incorrect payments on bills with
a January service date. |
