Advocate Lutheran General Hospital offers ‘Living with Heart Failure’ education and support group |

Being diagnosed with heart failure can raise more questions than answers about how to live life and manage the disease. Advocate Lutheran General Hospital has developed a new education and support group designed to provide patients and their families an avenue to understand how to live with a chronic disease.
The post-hospitalization program, Having a Future with Heart Failure, takes a multidisciplinary approach to the management of acute and chronic heart failure care. Health professionals involved in the program include a heart failure advanced practice nurse (APN), dietitian, pharmacist, exercise physiologist and ethicist.
It is estimated that nearly 5 million Americans have heart failure. Fortunately, many factors that can prevent heart failure and improve outcome can be applied at any stage.
“Education is key,” said Jean Mau, APN, heart failure program director. “You can’t reverse many conditions that lead to heart failure, but heart failure can often be treated with good results. Medications improve the signs and symptoms of heart failure and lifestyle changes such as eating the right foods, monitoring daily weights and taking an active partnership you’re your health care team can all help improve quality of life.”
A series of weekly, outpatient sessions include topics of living with heart failure, heart failure medications, dietary guidelines, understanding the role of exercise, the psychological side of heart failure and ongoing heart failure management. The program is funded by a grant from the Advocate Lutheran General Health Partners Endowment.
