** BRIDGTON HOSPITAL AWARDED $50,000 GRANT **
MeHAF Grant to Improve Patient Safety
Bridgton Hospital, a Critical Access Hospital and member of the Central Maine Medical Family, announced that it has received a $50,000 grant from the Maine Health Access Foundation (MeHAF) to improve its systems for managing medications for better patient safety. The funding follows an earlier grant awarded to Bridgton Hospital to assess their strengths and weaknesses and identify ways to improve how the hospital administers records and educates patients about medications both during hospital stays and upon discharge. National studies indicate that misuse of medication is a leading cause of emergency room visits, hospital admissions and readmissions.
Bridgton Hospital staff working on the initiative included project director, Bobbi Cribby, RN, BSN, Director of Clinical Practice, along with Denyell Gerchman, R Ph, Pharmacist, Charles Frost, Pharm. D., Pharmacist,
Michael Lessard, R Ph, MBA, Director of the Pharmacy, and a multi-disciplinary team of healthcare professionals.
According to Mrs. Cribby, “Studies show that better verification, clarifying individual medication histories and improved documentation when in the hospital - also known as medication reconciliation - can dramatically reduce medication errors and the costs of readmission. We will be working closely with providers of patient care in our surrounding communities, such as physician offices, local pharmacies, home health agencies and nursing homes. There will be a common goal of accurate, current medication lists available, providing the safest care for our patients.” She continued, “While patient accountability is important, of course, the team will be making significant outreach efforts to local community and civic groups, over the months ahead, to educate the public on the importance of being knowledgeable related to the medications they are prescribed.”
“We are grateful for MeHAF support in studying and designing an improved medication management system at Bridgton Hospital and we value collaborating with the 13 other Critical Access Hospitals in Maine that have participated in this work. We have, and will continue, to learn a lot great deal from other’s experiences and have covered a lot of ground together that would have taken much longer to accomplish as individuals,” notes Mrs. Cribby. The Bridgton Hospital project is entitled “Medication Reconciliation with Emphasis on Patient Education and Community Partners Involvement.
